**Designed for clients needing extensive or around-the-clock care.
📝 Comprehensive Assessments
- Performed before services begin.
- Conducted by a licensed Registered Nurse (RN).
- Includes full evaluation of the client’s physical, emotional, and functional needs.
- Helps determine the customized care plan and appropriate level of caregiver support.
Reference: 12VAC5-381-360(C) – “Such services shall be delivered based on a written plan of services developed by a registered nurse… reassessed as often as necessary, but at least every 90 days.”
✅ Supervisory Visits
- Conducted every 90 days or sooner if there’s a change in the client’s condition.
- Ensures the care plan stays aligned with the client’s evolving needs.
- Identifies any new safety concerns or health risks.
